If you are medical practitioner of an ex-patient of Dr Timothy McDonald (previously practicing in East Maitland, NSW), you may submit a request for a copy of any historical medical records that are held on file. All details must be completed for a valid request. Please allow 2-4 weeks minimum for a response. For compliance purposes, follow up questions may be required to verify identity prior to release of information.
